My Parental Consent Form

The Parental Consent Form must be completed by the legal guardian at the time of registration. The legal guardian must take responsibility for updating the details as soon as any changes occur. The questions are set by the organization's administrators and some answers are likely to be compulsary. The questionnaire should contain a question of the type 'Please enter today's date' so that the administrators can check when the form was last updated by the legal guardian. Once a year the legal guardian should be required to check the details; once checked the signatory date can be updated.
